Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location. This Telehealth Informed Consent (“Consent”) explains the risks, benefits, and choices regarding telehealth services provided through Prime TRT LLC in partnership with our affiliated professional corporation (PC) groups and clinical network administrators, including OpenLoop Health.

1. Medical Providers & Clinical Services

• By consenting to treatment, you understand that your healthcare services will be delivered by Licensed Providers and Licensed Clinicians operating within our clinical network.

• These clinicians are independently responsible for the medical care, diagnoses, and treatment plans they provide to you.

2. Benefits of Telehealth

• Convenience: Access to medical evaluations, testosterone optimization therapy, and ongoing monitoring from your home without the need to travel.

• Efficiency: Secure digital transmission of your health data, intake questionnaires, and clinical history directly to your assigned provider.

3. Risks & Limitations of Telehealth

• Technical Delays: Electronic transmission of data could be disrupted or delayed by technical failures.

• Evaluation Limitations: A virtual evaluation relies heavily on the accurate information you provide in your intake questionnaire. In rare cases, a lack of an in-person physical exam may limit the provider’s ability to identify certain underlying conditions.

• Emergency Care: Telehealth is not for emergency medical situations. If you experience a medical emergency, you must call 911 or visit the nearest emergency room immediately.

4. Privacy & Security

• All communications, electronic medical records, and protected health information (PHI) transmitted through this telehealth platform are encrypted and fully compliant with HIPAA standards.

• Your clinical information will only be shared with your assigned Licensed Clinician, platform administrators, and our authorized Pharmacy Partners to fulfill your prescriptions.

5. Patient Acknowledgement & Consent

By clicking “I Agree” or proceeding to the clinical intake process, you acknowledge and agree to the following:

1. I have read and understood the information provided in this Telehealth Informed Consent.

2. I consent to receiving telehealth evaluations and treatment from Licensed Providers within the affiliated network.

3. I understand that I can withdraw this consent at any time by pausing or cancelling my services through the patient portal.
